The standard way to create PDF forms is to first create the base document (in whatever layout software you have), then produce a good, compact PDF. Then take this PDF into Document Editor (Pro) and continue by adding fields and logic. As you state CS6, you will have Document Editor X. There you will have to use the Forms Edit mode to add fields and logic. This is normally sufficient. For a smart form, you will need Document Editor JavaScript.
